Enter your license type, license number, password (last four digits of your social security number) and click the Start Renewal Application button. A link will display to download & print your license. You may also print your receipt and application.
Select your license prefix and then add 7 digits of your license number. License Number Help

Practitioner licenses are now 8 characters for both the Practitioner Profile and License Renewals. For example:
Use an 8-character license number to log in to the Practitioner Profile Search. The profile will display the 8-character license number.
