A health care practitioner who is newly licensed, has had their license reinstated, or has renewed their license can log in to the Practitioner Profile and download a digital copy of their license.

Steps to take if the practitioner is newly licensed and wants to set up a Practitioner Profile:

  • Click “New Registration” on the login page.
  • After setting up, print the license from the new Practitioner Profile page.

Steps to obtain a copy of the practitioner's license from the Practitioner Profile:

  • Log in to your “Practitioner Profile”.
  • Click on “Print Duplicate License” in Portal Navigation.

A health care practitioner can print a copy of their license during the renewal process and can print a copy from the renewal portal for 24 months after renewal. The practitioner may also obtain a copy of their license by downloading and printing it from the Practitioner Profile, or by obtaining a copy of their renewed license directly from the Board's website.

No, the Board will not mail hard copies of a license.

Yes, a health care practitioner can obtain a duplicate license by printing it from the Practitioner Profile if the license is on Probation status, if the practitioner was licensed through conceded eminence, or if the practitioner has a volunteer license. The process is not different.
